Care Gap Management
Our Care Gap program adds two more channels to reach your member, regular old US Mail and the member’s doctor.
As part of the tools available to both you and your case managers, maybe some members haven’t enrolled in HealthAssist, or they have, but still aren’t getting their regular preventative exams. Care Gap gives you another option; send them a letter.
The process is simple. The letters templates are ready and waiting, through HealthAssist. You select to send letters to the member or primary care physician or both.
